Important Registration Information
Payment Policy:
All registration fees must be paid in NZ Dollars. FULL PAYMENT must be made prior to attendance at the conference.
Cancellation Policy:
Cancellations must be made in writing.  All cancellations received by NZSDA on or before 24April 1998 will be entitled to receive a full refund.  Cancellations received by NZSDA after 24April 1998 will incur a $100 cancellation charge.  Cancellations must be made in writing.  Refunds will not be granted to registrants who cancel after 1 May 1998 or who fail to attend the event.  Substitutions for paid registrants may be made at any time.
Conference Hotel Information:
The 1998 NZSDA Conference Official Hotel is the Noahs Christchurch Hotel, Cnr Worcester Street and Oxford Terrace, Christchurch Central.  To book a room and to receive the special room rates, you MUST register directly via this form or by using the printed registration form available in the 1998 NZSDA Conference brochure or print and fill out a copy of this Hotel Reservation Form which can be submitted by fax or direct by e-mail.  All reservations must be submitted by 31 March 1998.
